Notification of authorization for CSSA services and forms (for an agent)
A natural person or business entity authorised by this form advises that it is authorised for all CSSA actions, services and forms or only for selected CSSA services and forms. At the same time, it is possible to use the form to designate employees as principals authorised to use selected CSSA services and forms intended for employers. At the same time, these designated employees will also be able to perform actions for you as an employer. Please note that if the principal is a self-employed person, your designated employees will be able to perform all electronic actions on their behalf.
It is possible to attach a copy of the power of attorney to the form, if it has not already been submitted to the DSSA earlier, or if it has not been registered by the principal via Administration of Powers of Attorney on the CSSA ePortal, available after logging in.
After sending, the completed form will be registered in the electronic file service, and the relevant DSSA will then create an authorisation in the CSSA system.
1. Please note that an authorized legal entity is not allowed to use the services of the ePortal on behalf of another entity, it is only allowed to send e-Submission forms and other forms.
2. In order to use the services of the CSSA ePortal for a principal (for example, Overview of temporary incapacity for work of employees), it is therefore necessary to designate a natural person (employee), who will then use the services according to the stipulated scope of authorization, using their own login:
- Citizen identities - eObčanka, NIA ID (formerly name/password/SMS), CA identity 1 with a Starcos card, MojeID, Bank identity or
- using its data box.
3. An authorized natural person or legal entity logged in to the CSSA ePortal can manage authorizations issued to their employees in the Administration of Powers of Attorney section.
