Application for an entry into the register of employers
The form is used by an employer who employs people to register in the register of employers.
On the form an employer it also enters all the financial institutions in the Czech Republic at which it maintains accounts and all its wages departments. In the case of the wages departments, it will state their addresses and the names of persons who are authorized to act for them on behalf of the employer.
The employer is also obliged to register each of its wages departments in the employers’ register. On the form, it must also indicate the range of employees, i.e. those employees for whom the wages department maintains records of wages or salaries.
