Request for determination of legislation applicable for an employed person, who is simultaneously self-employed (form)

With this form, a person performing employment and self-employment in two or more states requests the issue of a determination of legislation applicable, i.e. form A1 in the case of European Coordination regulations or a bilateral form/certificate in the case of an international social security treaty.

In the case of electronic submission of an application for the issue of a certificate of affiliation with legislation by sending an xml file to a data box, each application and its attachments must be sent separately, i.e. only one xml file and the relevant attachments may be sent in one data message.
