Request for cancellation of pension payment by bank transfer

With this form, a pension recipient (i.e. pensioner, legal representative, guardian, representative under Sections 49 and 50 of the Civil Code or special beneficiary) requests the cancellation of pension payments to an account in the Czech Republic (under Section 64(1) of Act No 155/1995) or with it the owner of an account in the Czech Republic requests the cancellation of the payment of a spouse's pension to his/her account.

The pension recipient can send the form electronically after logging in to the CSSA ePortal or via their data box, or submit the handwritten form in writing at any DSSA.
