Request for Informative personal record of pension insurance (form)

The form is used to submit a client's request for sending an Request for Informative personal record of pension insurance. Thanks to it, the citizen will get an overview of the pension insurance (employment) periods, or replacement periods of insurance, stored in the register of the CSSA, since 1986 also an overview of the assessment bases and excluded periods. You have the right to process an information sheet once per calendar year.

The Informative Record will be drawn up within 90 days from the date of delivery of the application. You can find more about the Informative Record in the document Explanation of the Informative Personal Record of Pension Insurance (PDF 451,5 kB).
