Application for setting up of pension payment by bank transfer to an account in the Czech Republic - account owner

The form is used to exercise a client's request to set up pension payment by bank transfer to the account of a natural person in the Czech Republic (pursuant to Section 64(2) of Act No 155/1995). It is also intended for submitting a request to change a bank/account number for the payment of the account holder's pension. The form can also be used by other persons (see Section C and D).

The details of the owner's account number must be verified and confirmed by the bank or savings and loan union. The form so confirmed and signed may be submitted in writing at any DSSA.
